| Your child’s welfare is of
the utmost importance to us at the Kinkan Dojo, and these terms
and conditions have been put in place to ensure your children
can practice martial arts in as safe an environment as possible. |

For the purposes of the Application
Form “STUDENT”
or “STUDENT’S”
refers to the child who is participating in the training programme
and who is under the care of the signatory on the Application
Form.
The “PARENT”
or “PARENT’S”
refers to the parents or carer who has full parental responsibility
for the child undertaking the training programme. |

these terms and conditions, please do not hesitate to speak
to one of our instructors. |

- In all classes, demonstrations and activities conducted/organised
by the Kinkan Dojo and its instructors, reasonable care
is taken to prevent serious injury and minimise accidents.
The PARENT states that
he/she is fully aware that martial arts training, even under
the safest conditions possible, may be dangerous.
The PARENT hereby agrees
to accept full responsibility and assume all risks, including
those caused by acts of God, of injury, death and/or loss
to his/her person and/or property, knowingly and/or voluntary.
- The STUDENT/PARENT
agrees to obey all rules and regulations that the Kinkan
Dojo puts into place in order to minimise these risks.
- The STUDENT/PARENT
knowingly, voluntarily and irrevocably waives any and all
past, present and future claims, demands and causes of action
which the STUDENT/PARENT
now has or may have in the future against all instructors,
assistant instructors, organisers and/or students, while
on the premises hired/borrowed/used by the Kinkan Dojo for
martial arts instruction, for any and all past, present
and/or future injuries, death, loss, including those caused
by acts of God, received while participating in activities
conducted by the Kinkan Dojo as a student, participant,
spectator and/or visitor, or in any other manner or form,
taking part in the exercises, practices, excursions, and/or
demonstrations of said martial arts training.
- The PARENT certifies
that the STUDENT is
physically and mentally capable to participate in the said
martial arts training, despite the rigours and dangers inherent
in such an undertaking
- It is the responsibility of the PARENT
to ensure that they have adequate personal insurance as
well as cover for the loss of or damage to the STUDENT’S
personal property
- If the PARENT is
going to be delayed and cannot collect the STUDENT
on time, the PARENT
must contact Jason
or any of the assistant instructors to inform them of this.
If the PARENT needs
to send someone else to collect the STUDENT,
it is best if this person visits the class and is introduced
to the instructors before such a need arises. PLEASE
NOTE THE FOLLOWING:
- the instructors will not
release the STUDENT
into the care of any individual they do not believe
to have any connection with the STUDENT,
or to any individual under the age of 14;
- the PARENT
must collect the STUDENT
on time, i.e. at the time that the class finishes;
- if the PARENT
is late on more than 3 occasions to collect the STUDENT,
then the instructors reserve the right to make a charge
of £1 per minute of lateness.
The instructors also reserve the right to terminate
the training placement for persistent late collection.
- FEES:
For each STUDENT
the fee for the first 4 weeks of training is payable weekly
before the class starts.
If the STUDENT wishes
to continue training after this 4 week period the fees then
become payable 4 weeks in advance, at the first class of
the month.
Refunds for absence will not be made.
- The instructors of the Kinkan Dojo reserve the right to
decline an application without having to state a reason.
The instructor’s decision will be final in all cases.
- These Terms & Conditions are subject to alteration
by the Kinkan Dojo.
- The PARENT’S
signature on the Application Form indicates the PARENT’S
acceptance of these Terms & Conditions and the PARENT’S
desire for the STUDENT
to participate in this martial arts training programme.
